The California Department of Public Health is staffing a phone hotline to answer the public’s questions about COVID 19 and vaccines.

Authorization for Release of Information Form

If you are struggling to get a resolution to a claim or get timely assistance from a state agency, such as the California Employment Development Department, the California DMV, or the California Department of Consumer Affairs, my staff may be able to help you. In order for my team to best assist you, we ask that you fill out the following Authorization for Release of Information. This form gives us the information we need to speak to agencies on your behalf.

You can fill out all of the information in this editable online form, save it, and email it to our team at senator.atkins@senate.ca.gov.
